INSTRUCTIONS: How to download and complete the HTFCU ATM/Debit Card Application Form.

You have 3 Choices.

Choice #1:   

Double click on the Microsoft Word Icon, fill out the form by using the tab-key to move from field to field and press spacebar to put check in boxes. Type the data into each  field, print, sign the ATM/Debit Card application form, and mail the form to HTFCU.

Double click on the Microsoft Word Icon, Print the form and fill the payroll deduction form out by hand.  Sign the form and mail the ATM/Debit Card form to HTFCU.

Double click the PDF File Form which will open the PDF Form in Adobe Acrobat and print the form. Fill the form out by hand, sign it and mail to HTFCU.
